North Central Health District Administering Shot to TC Students

On Tuesday, November 23, 2021, the North Central District Health Department (NCDHD) will be on site in each of the schools to administer Covid Vaccines and/or Flu Shots to Trimble County Students. Moderna, Pfizer and J&J will be available for students ages 18 and up and Pfizer will available for students 5 yrs. of age and up. NCDHD staff will be at the school to administer dose 1 and/or dose 2. In addition to Covid vaccines, they will also be providing flu shots to students of any age. Vaccinations will take place at MES 8:30am - 10am, BES 10:30am-12:00pm and TC Jr/Sr High 1:00pm-2:30pm. A parent/guardian MUST be on site, at school with the student, in order for them to be vaccinated.

If you are requesting a vaccine for more than one child, please fill out a separate form for each child that will be vaccinated.
