Southwestern Athletic Department to Offer Student Physicals

Physicals will be done during school day from 11am-5pm in High School Gym

The Athletic Department and Foundations Family Medicine have partnered up to offer physicals at Southwestern High School on May 17 from 11AM-5PM. We will be working on a schedule for classes to come down to the gym to do this during the school day without interruption to any finals that are being done that day. Students will need to get a physical form or pick one up that day. Part of the form must be filled out before the physical, so it will speed things up if the student has one prior to arrival. The physical that you get that day will work for all sports next school year. Cost is $20, but if you have multiple kids in the family, it will be $20 for the first one and $10 for each additional child. If money is an issue, just talk to Mr. Scroggins and something can be worked out with no issue. The offices will have physical forms closer to May 17 for you to use.
